Searching for self-directed community habilitation direct support professional to work with a person to work full-time hours. Experience with providing direct care/ direct support for at least 2 years requested. Hourly wage is up to $20.00. Mileage and activity fee reimbursement included while providing community habilitation services. Smartphone access necessary to clock in and out. Staff must have access to reliable transportation and a driver's license.

• Under the direction of the Person Supported and Fiscal Intermediary, this position is responsible for providing person directed services and supports as outlined in the person’s Self-Directed Budget, Life Plan, and Staff Action Plan(s).

Education:

• High School diploma, GED, or an acceptable TABE test score is preferred.
