Diabetes Coordinator / Educator Job at Memorial Healthcare
JOB SUMMARY
Under general supervision is responsible for the planning, implementation and evaluation of the Diabetes Self-Management Learning Program (SMLP) in accordance with regulatory standards. Coordinates, plans and evaluates diabetes related educational offerings for the community and for hospital staff as appropriate. Functions as a diabetes instructor by assessing patient/caregiver educational needs, planning, implementing and evaluating diabetes self-management training in individual and group settings. Coordinates the activities and functions of the Diabetes and Out-Patient Nutrition Counseling Center (DNCC) in accordance with the organizational and nursing service policies and procedures. Strives for superior performance by consistently providing a product or service to leadership and staff that is recognized as ultimately contributing to the patient and family experience. Recognizes and demonstrates understanding of patient and family centered care.
PRIMARY JOB RESPONSIBILITIES
- Coordinates plans and evaluates all services and programs offered in the DNCC.
- Maintains current knowledge of state and national regulatory standards, rules, requirements and guidelines that affect the Diabetes SMLP and assures that activities, processes, procedures and functions of the program are in compliance.
- Coordinates and chairs the activities of the Diabetes Advisory Committee including the annual program review and planning process.
- Works with and guides program instructors to implement the annual plan, achieve program goals, improve program processes and outcomes and provide quality service to patients/caregivers and referral sources.
- Oversees and coordinates program data collection and evaluation, outcome measurements and evaluation and CQI data and evaluation. Reports data to the AVP- NOW.
- Prepares the annual program review/annual plan and annual status reports for regulatory agencies and files those with the appropriate agencies at the required times.
- Participates in the preparation of the budget.
- Coordinates the receipt and use of the Diabetes Special Fund with the Executive Director of the Memorial Healthcare Foundation.
- Provides supervision, performance evaluation and direction for the DNCC and SMLP staff and coordinates and provides orientation for new staff members.
- Serves as a liaison between the program staff, the Advisory Committee and hospital administration
- Develops, maintains and updates policies, procedures and operating instructions for all programs in the DNCC.
- Coordinates education programs, projects and activities with the Education Department to update and enhance hospital staff knowledge regarding quality diabetes care for in-patients. Serves as a resource regarding diabetes care for community groups and agencies and for other healthcare agencies and individuals.
- Demonstrates job related aspects of patient safety to assure a safe environment, safe and effective use of equipment and technology, decreased risk of potential adverse patient occurrences.
- Demonstrates knowledge of and supports hospital mission, vision, value statements, standards, policies and procedures, operating instructions, confidentiality statements, corporate compliance plan, customer service standards, and the code of ethical behavior.
DEPARTMENTAL AND ADDITIONAL JOB RESPONSIBILITIES
- Promotes and coordinates continuing education opportunities for the program staff and communicates changes and updates regarding regulatory standards, research findings and process changes.
- Works with Patient Accounts and Memorial Medical Associates regarding insurance, billing and reimbursement issues and processes.
- Coordinates marketing strategies to promote all DNCC services with the Business Development department. As appropriate, responds to requests for participation in community events and offerings

JOB SPECIFICATIONS
EDUCATION
- Graduate of an accredited school of nursing, current license as a registered nurse in the state of Michigan or graduate of an accredited clinical dietetics program, with current license as a Registered Dietitian in the state of Michigan.
- Must hold a current status as a Certified Diabetes Educator. Baccalaureate degree required, Master’s degree preferred. Current certification in Basic Life Support required.
- AHA Healthcare Provider BLS certification.
EXPERIENCE
- Five years’ experience within the clinical healthcare area and two years’ experience in patient teaching, community or health education required.
- Two years academic or experiential preparation in chronic disease care and education and in program management required.
- Effective in verbal and non-verbal communication skills. Good interpersonal skills and effective customer service skills.
- Remains current in wellness, diabetes education, patient teaching and program management skills.
ESSENTIAL PHYSICAL ABILITIES/MOTOR SKILLS
- Able to travel independently throughout all Memorial Healthcare facilities.
- Small motor skills required for operating modern computer, office, and telephone equipment as utilized by Memorial Healthcare (MHC).
ESSENTIAL TECHNICAL ABILITIES
- Proficiency using modern office, computer and telephone equipment as used by Memorial Healthcare.
ESSENTIAL MENTAL ABILITIES
- Ability to adapt and maintain focus in fast paced, quickly changing or stressful situations.
- Ability to read and interpret a variety of documents including, but not limited to policies, operating instructions, white papers, regulations, rules and laws.
ESSENTIAL SENSORY REQUIREMENTS
- Able to see for the purpose of reading information received in formats including but not limited to paper, computer, reports, bulletins, updates, manuals.
- Able to hear for work-related purposes.
