Coordinator of Care Management Job at DHA - Valley Regional Hospital
The Coordinator of Care Management will be responsible for assuring the receipt of high quality, cost efficient care for all patients who seek services at Valley Regional Hospital; the oversight of social work, case management, SNF referral management, utilization review and discharge planning; developing expected patient outcomes, defining appropriate resource utilization and works collaboratively with providers to set goals; assuring the department’s work and hospital compliance is in line with federal and state regulations, reimbursement guidelines as well as the policies and procedures of Valley Regional Hospital.
The qualified candidate must hold a Masters Degree in Social Work or a current state license as a Registered Nurse. Certification in Utilization Review or Case Management is a plus.
Experience
Minimum of 5-7 years experience as a case manager or utilization review nurse in a healthcare setting, with a comprehensive understanding of insurances and state and federal regulations governing Critical Access Hospitals.
