Job Summary:
Working within a wrap-around service model, connect social service agencies and basic needs providers with the school district and with families in need of accessing appropriate resources.
Roles/Responsibilities/Abilities:
- Connect students/families to community resources and information. Provide referrals for community services, support access to local resources and medical services, assist with completing paperwork for services
- Create a network of resources, in coordination with the school and social service agencies, to ensure family access to available services
- Provide CRS overview presentations and staff training as needed
- Assist with insurance providers/paperwork, medical care providers and/or ensuring access to the appointment
- Establish connection between academic success and basic needs by working to remove barriers preventing families from accessing and/or using the available resources
- Serve as a liaison, coordinating and aligning services between the school, family, Family Resource Center, McKinney-Vento and/or social services
- Seek out community resources and increased coordination of services provided to families
- Respond to general inquiries and/or concerns by applying problem solving strategies that result in positive resolution
- Meet with the family of each new student enrolled to foster a sense of belonging and begin to build a lasting home/school partnership. And, assess the student/family needs and plan for supports, accordingly.
- Professionally represent the school and the District in interactions with parents, community, staff, and students
- Interact and develop working relationships with professionals, staff, children and their families using positive interpersonal communication
- Maintain cultural sensitivity when working with families and students
- Communicate clearly and concisely both orally and in writing
- Possess general knowledge of computer usage and utilize database and internet software, spreadsheets, word processing, Google suite and email
- Work without technical supervision and take responsibility for job duties; exhibit effective time management
- Ability to work flexible hours, as needed
- Must be self-motivated and reliable
- Ability to maintain confidentiality with discretion
- Maintain records and prepare weekly/monthly reports, as directed by Supervisors
- Read, interpret and follow District rules, regulations, policies and procedures
- Attend work regularly and be punctual
- Attend staff meetings, as well as other required meetings
- Maintain appropriate certifications and training hours as required
- Perform all other job-related duties as assigned by immediate Supervisor and/or Family Resource Coordinator
- Prepare informational materials regarding CRS services and participation in school specific events

Help families in connecting with, and utilizing resources, including scheduling appointments, and assisting with completion of applications for programs for which they may be eligible

Share information regarding available resources to school staff and/or families

Facilitate communication and coordinate services between providers, the school and families

Assist families/staff with completion of paperwork to support access of community and/or school based resources

Communicate with county and community based agencies/professionals

Expand knowledge and understanding of community resources, services and programs provided

Provide training and/or professional development to staff; provide presentations to individuals and/or small groups
1-2 times per quarter, as needed
Identify, and be available, during ‘office hours’ to respond to parent and/or staff needs

Check, and return, emails and phone message on a regular basis

Follow up with families/students, who have received services, using a variety of mediums [e.g. phone, virtual, in person, email] to check on progress, answer questions, adjust supports, make new connections, etc.

Attend regularly scheduled Community Resource Specialist meetings, trainings and professional development session

Gather and provide data reports relative to assistance/connections and maintain weekly/monthly activity logs as directed
