GENERAL PURPOSE:

The Community Resource Liaison (CRL) position is the result of public and private contributions and interests in creating a resource to the community of Boulder City (including Boulder City Police and Fire) and developing hospital-based programs and services.

ESSENTIAL FUNCTIONS:

As a community resource, the CRL will be a direct resource for helping members of the community, patients and their families navigate and access community services, other resources, and adopt healthy behaviors.

The CRL also supports providers and the Case Managers and other hospital development personnel through an integrated approach to care management, community outreach and referral development.

CRL provides assessment, social support and informal counseling, advocates for individuals and community health needs, and may provide other services such as education and training for hospital employees, Boulder City Police and Fire and the public.

DUTIES AND RESPONSIBILITIES:

  • Establish excellent working relations and liaise with community stakeholders (Nevadans for the Common Good, BCPD, BCFD, Senior Services, Lend-a-hand, etc.)
  • Provide accessibility, follow-up and goal setting with clients/patients/families.
  • Follow – up via phone calls, home visits, E-visits and visits to other community settings where patients can be found.
  • Provide referrals for services to community agencies as appropriate.
  • Help patients connect with transportation resources and provide appointment

reminders in special circumstances.

  • Represent the BCH brand’s mission, vision and values
  • Work closely with medical providers to help ensure that patients have comprehensive

and coordinated care plans.

  • Work collaboratively with other clinical personnel assigned to the same patient.
  • Be knowledgeable about community resources appropriate to needs of patients/families.
  • Responsible for providing consistent communication to the Case Manager to evaluate

patient/family status, ensuring that provided information and reports clearly describe

progress.

  • Record patient care management information in the EMR as appropriate.
  • Attend regular staff meetings, meetings as requested and complete required training.
  • Maintain HIPPA compliance at all times.

EDUCATION AND EXPERIENCE REQUIRED:

· Masters degree in Social Work required.

  • 3 to 5 years in a healthcare setting assessing and triaging patients.
  • Medical terminology and/or background required.
  • Written and oral fluency in English and Spanish is preferred.
  • Experience working in a multi-cultural setting.
  • Experience working in a community-based setting for at least 1 to 2 years preferred.
  • Basic computer skills required; electronic medical record (EMR) experience preferred.
  • Understand the community served, community connectedness.
  • Good communication skills, such as listening well, and using language appropriately.
  • Ability and willingness to provide emotional support, encouragement and motivation to

patients.for

Job Type: Full-time

Pay: $29.00 - $40.80 per hour
