Community Resource Coordinator- OPH Region 6
Goal
The Louisiana Department of Health has implemented a high priority healthy living initiative named Well-
Ahead. The goal of Well-Ahead is to promote healthy behaviors (such as tobacco-free living, healthy eating, physical activity, and healthy work/life balance), all within the spaces and places in which we live and work every day. To this end, Well-Ahead is focused on developing a plan for increased coordination of resources and targeted investments in order to make it easier for the residents of Louisiana to make healthier choices.
Through regional community resource coordination, Well-Ahead aims to establish community partnerships with the goal of improving heart disease and diabetes prevention and management practices. The
Community Resource Coordinator will identify and coordinate community-based resources and implement systems and programs that promote, maintain, and improve individual and community health.
Scope of Work
I. Establish Chronic Disease Prevention and Management Programs - 65% effort
a. Meet Chronic Disease Prevention and Management targets set by Well-Ahead: i. 3 new community-based or healthcare organizations implementing the National DPP.
ii. 2 existing National DPP receiving value-based payments based on target enrollment, retention, and weight loss goals.
iii. 2 healthcare organizations (including community pharmacies) meeting standards to achieve ADCES-accreditation/ADA-recognition for Diabetes Self-Management
Education and Support (DSMES) programming
iv. 20 new community or faith-based organizations participating in SMBP program with a clinical connection for referrals.
b. Outreach Strategy Development
i. Collaborate with regional Well-Ahead Representative to develop a coordinated outreach strategy to regional WellSpots for the purpose of implementing chronic disease prevention and management programs.
ii. Collaborate with the Well-Ahead Diabetes and Heart Disease Program Managers to continually work to improve and modify the outreach strategy. iii. Engage with local and regional Healthy Community Coalitions (HCC) to expand knowledge of community resources and increase opportunities to identify non-traditional screening settings
c. Technical Assistance
i. Provide technical assistance to community-based and healthcare organizations in Region 6 in the development of DSMES, National DPP and SMBP programs. This includes, but is not limited to:
1. Directly assisting organizations in implementing National DPP, SMBP, and DSMES programs.
2. Connecting organizations with technical assistance providers for the implementation of chronic disease prevention and management programs when appropriate (ex.Well-Ahead staff member, partner organization).
3. Provide training on SMBP best practices including how to properly conduct SMBP screenings.
4. Provide technical assistance to CBO’s with screening barriers to ensure deliverables and screening goals are met.
5. Directing organizations to online resources for program implementation.
ii. Maintain subject-matter expertise on implementation and delivery of chronic disease prevention and management programs. This includes, but is not limited to:
1. Attaining National DPP Lifestyle Coach Certification.
2. Participation in DSMES and National DPP State Quality Specialist training, as available.
3. Completion of AHA/AMA Target BP training
4. Participation in programmatic training from Diabetes and Heart Disease Program
Managers as needed.
d. Reporting/Data
i. Submit at least 1 CBO success story per quarter to Well-Ahead Heart Disease Program
Manager and Diabetes Program Manager via a provided online form. All success stories should include quotes from management and employees, and supplemental photos. The
CONTRACTOR shall have the author of each quote or testimonial sign a release allowing
Well-Ahead to use their statement publicly for promotion of the initiative. ii. Update chronic disease program trackers weekly, at minimum. iii. Participate in calls with Well-Ahead Diabetes and Heart Disease Program Managers a minimum of once/month. iv. Email successes to the Well-Ahead Diabetes and Heart Disease Program Managers as they occur for the purpose of sharing in real time with upper management. v. Submit invoices and program reports as requested. Invoices should be submitted on or before the 1st and 15th of each month.
e. Communications i. Regularly promote Well-Ahead through earned media opportunities including but not limited to press releases, radio interviews, YouTube features, and public relations pitches to local news networks with prior approval of Well-Ahead. ii. Represent Well-Ahead (through presentations or attendance) at conferences, meetings, and other environments as required or as opportunity allows. f. Online Resource Management i. Continue to add new resources and maintain the accuracy of current resources in the
Community Resource Guide in assigned region. This includes a minimum of 5 new resources in the region in addition to newly development prevention and management programs during the contract term. ii. Maintain comprehensive knowledge of all resources for chronic disease prevention and management program implementation available at wellaheadla.com and on shared drive.
II. Clinical Linkages and Connection - 30%
a. Partner Development/Training i. Maintain current Well-Ahead partnerships and develop new partnerships for the purpose of reducing health disparities through the implementation of chronic disease prevention and management programs. This includes, but is not limited to:
1. Increasing awareness of all Well-Ahead educational platforms and collaboratives including but not limited to WALPEN, LaDEN, LaDEN Prevents, and the Louisiana
Obesity and Diabetes Collaborative to ensure understanding and enrollment in the professional networks.
2. Communicate with the Well-Ahead and/or Regional Representatives regarding needs for any partnership introductions, resource development, hosting regional presentations, or other necessary tools needed to successfully implement action plan.
3. Maintain current and up-to-date knowledge of Well-Ahead programmatic efforts and best practices for providing technical assistance with implementation of efforts specific to Chronic Disease.
4. Collaborate with Well-Ahead Representatives to increase WellSpot benchmark achievement related to heart disease and diabetes management and prevention.
5. Engage with local and regional HCC to expand knowledge of community resources and increase opportunities to identify non-traditional screening settings. Establish a relationship with every HCC in assigned region and attend a minimum of one coalition meeting per month. ii. Establish and maintain partnerships for the purpose of increasing clinical interventions that increase access to community resources and lessens the burden of chronic disease in the region. This may include, but is not limited to:
1. Outreach to healthcare organizations and provide information on National DPP,
DSMES, and SMBP programs. Support interested healthcare organizations in establishing services. This includes providing information on the accreditation process and/or implementation processes. As warranted connect organizations with the Diabetes and/or Heart Disease Program Managers for additional guidance.
2. Coordinate with and Well-Ahead Representatives to educate healthcare providers on available community resources and referral processes to those resources including National DPPs, SMBP programs and others.
3. Assist sites in the development of formal or systematic bi-directional referrals between healthcare organizations and community-based organizations using tools such as HELM and templates provided by the Well-Ahead. The established system or process for closing the loop on bi-directional referrals can be via phone, fax and/or electronic means.
III. Other - 5% a. Attend all Well-Ahead trainings and meetings as requested, including attending Well-Ahead team meetings.
