Lirio Resources is looking for a qualified Community Nurse who will be responsible for the medical oversight of all individuals served through Lirio Resources. As Community Nurse, you will be responsible for maintaining clinical compliance for SGL, SFC, Wellness Coordination, and Supported Living homes in their assigned territory. The Community Nurse will perform all required duties associated with wellness coordination in addition to administering staff trainings, teaching medication administration, performing medication observations, generating monthly and quarterly summaries, as well as all additional tasks assigned to you by the Directors. All services are to be delivered in such a manner that meets all federal, state, local, and accrediting entity regulatory requirements as well as Lirio policies and procedures.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Coordinate physician and medical services for individuals.

Perform routine visits to individuals served in accordance with requirements of their service.

Perform routine medical audits for quality control. Identify, resolve and follow up on all issues and concerns within a timely manner.

Responsible for ensuring that medical services are delivered within all applicable regulatory requirements as well as company policy and procedure.

Actively participate as member of each individual’s IDT, providing relevant feedback and input related to the individual’s progress, supports, and needs.

Ensure that those served attend all scheduled appointments and follow-ups, maintain documentation of all appointments, ensure that a COD and 450B are on file annually for each person served.

Work directly with physicians, specialists, and pharmacists to ensure that the medical needs of those served are met.

Maintain and review individual medical records to monitor pertinent issues and provide thorough documentation, including preparation of any related reports.

Facilitate the training and orientation of new staff regarding medical procedures, health related in services, and delegation of nursing tasks.

Order necessary medication and supplies for individuals served.

Complete the coordination of the medication administration certification process.

Establish working relationships with community medical services, nutrition services, and behavior specialists to provide quality care.

Perform individual assessments and scheduled visits as per the requirements of each service delivered to that individual.

Regularly and effectively collaborate with other department supervisors.

Any other duties as assigned.

QUALIFICATIONS

No criminal history involving fraudulent, neglectful, exploitive or abusive activities related to individuals.
  • Knowledge of state medical regulations and procedures.
  • Knowledge of nursing practices and health care medical procedures.
  • Skill in administering CPR and first aid.
  • Ability to maintain confidentiality.
  • Ability to read and understand policies and procedures.
  • Ability to read medical records and related information.
  • Decision-making skills required.

  • Ability to travel to perform required visits and follow-up.
  • Ability to adapt to changing demands.
  • Ability to maintain a patient and positive attitude.

EDUCATION AND EXPERIENCE

  • Associates degree in nursing required with a minimum of two years of nursing experience.
  • Current Indiana license in good standing as a Registered Nurse or LPN.

Pay: $20.00 - $25.00 per hour
