OVERVIEW:
The Community Liaison is responsible for maintaining and promoting relationships within Eden Health integrated delivery system and patients.

EDUCATION AND EXPERIENCE
- 1-2 years’ experience in the home health or healthcare industry required.
- In depth knowledge of state and government regulations.
- Maintains regulatory requirements, including all federal, state, local regulations, and accrediting organization standards.
- Exceptional verbal and written communication and presentation skills. Bilingual a plus
- Self-starter
- Clinical background preferred, but not required.
- Knowledge of duties and functions of the home health Agency and its relationship to the healthcare community.
- Must have proof of up to date COVID vaccine, or submit an exemption request to be reviewed/approved prior to hire
