ESSENTIAL JOB FUNCTIONS:
- Identify and engage patients to maximize primary care, behavioral health, and other health services at GPH and through its community partners.
- Outreach to underserved individuals and families in the community to facilitate enrollment in healthcare at GPH and connect individuals with appropriate health insurance options, such as MaineCare, Medicare, the Health Insurance Marketplace and hospital Free Care programs.
- Help patients navigate their visits at Greater Portland Health, providing help and support before, during and after visits.
- Provide case management services, support individuals with their care plan, and develop strategies to improve their health and well-being.
- Coordinate with area housing authorities, schools, social service agencies, shelters, homeless street outreach teams, faith-based organizations, food kitchens, transportation systems, and other organizations to connect patients to needed resources and maximize utilization of GPH services for underserved populations.
- Follow-up with patient’s plan, including helping patients make appointments; complete lab tests; obtain referrals, imaging, and medications; find transportation, and utilize community resources.
- Assist patients in filling out forms including GPH forms, General Assistance, and housing and social security applications.
- Build understanding and social capital to support healthier behaviors and lifestyle
- choices.
- Deliver health information and community education sessions using culturally appropriate terms and concepts.
- Plan and attend outreach events in the community to inform individuals, families and community members about available services and programs at GPH.
- May, on occasion, translate and interpret for clients and providers if certified in medical interpreting.
- Serve as a liaison and link between GPH and other health and social service agencies and the community regarding COVID-19 testing.
- Provide and conduct COVID-19 testing, supply ordering, and lab support.
- Reach out to patients who are not engaging in care to set up appointments and gather information to support the patient.
- Support care team members in administrative tasks which improve patient access to care and enable patients to follow through on their care plan.
- Responsible for taking reasonable care of their own health and safety at work.
- Perform other duties as assigned.
