As a Community Health Outreach Specialist/Advocate for Johns Hopkins HealthCare, you are the conduit between Priority Partners and the community-based organizations that focus on the health and wellness of current and potential members. You enjoy being on the road in the Upper Eastern Shore area (to include Talbot, Caroline, Queen Anne's, Kent, and lower part of Cecil County), participating in events that promote the health and well-being of the community and grow and retain our Priority Partners membership.
What you'll do:
Collaborate with community-based organizations focused on health issues to coordinate resources and expedite promotional and informational events to meet the needs of current and potential Priority Partners members
Build and maintain community relationships throughout the Upper Eastern Shore in order to promote Priority Partners
Provide comprehensive outreach including a visible and active presence within the community to offer education as well as assistance with referral and enrollment
Participate in activities and functions directed toward engaging potential members and providers
Conducts presentations and attends health events to promote Priority Partner services in Upper Eastern Shore
You will be responsible for assisting all Priority Partners members with the following activities, including, but not limited to:
Redetermination of their health benefits
Change of address
Change of PCP
ID card issues
Complete required reporting
Keep current with State of Maryland enrollment procedures for the Health Choice and Maryland Health Connections
What you'll bring:
3+ years' experience in the community health arena
Community health education experience is highly preferred
Community health worker experience is preferred
Requires basic computer skills and the ability to use online calendars, email, and components of the Microsoft Office suite.
Requires interpersonal and communication skills to efficiently work with diverse groups of community members and to coordinate team members.
Requires excellent organizational/administrative skills, flexibility, and the ability to get things done. Requires the ability to demonstrate the skills for planning for development and evaluation of health education programs for the Medicaid and MCHIP population.
Requires the ability to adapt instructional skills for members with learning impairment, low literacy skills, or cognitive dysfunction. Requires the ability to establish and maintain effective working relationships with both internal and external customers and analytical skills to effectively assess and evaluate the health education needs of these customers. Requires the demonstrated ability to work independently and efficiently.
Basic knowledge of Medicaid and Medical Terminology is highly preferred
Bachelor's degree in Health Sciences, Education, Marketing/Communication
Work experience may substitute for education
Requires a valid Maryland driver's license and appropriate automobile insurance for consistent and dependable local transportation.
This position is on the road 80% of the time during defined busy season (April - November)
Requires the ability to store promotional items in your home office.
This position is remote, but you must live in the defined area.
