Community Health Navigator Job at DHA - Valley Regional Hospital
Valley Regional Healthcare is actively seeking a Community Health Navigator who will be responsible for guiding patients to obtain the most appropriate health and wellness services for their individual needs.
This important role advances our mission of improving the health and wellbeing of our community by looking at the whole person; addressing the specific needs of each individual and supporting those with complex conditions and/or socio-economic barriers to accessing health services. The right candidate will possess the ability to plan and implement initiatives to reduce or eliminate these barriers for patients.
VRH is looking for someone to provide short term care coordination, connection to resources and education, and support to patients. The ideal candidate will be able to work in collaboration with medical providers, primary care teams, and the outpatient social worker to facilitate, communicate and coordinate services on behalf of the patients. The candidate must be willing to obtain/maintain a familiarity with resources available to patients within the Greater Sullivan County area, and strong interpersonal and communication skills as they will be required to work effectively with a wide range of constituencies in a diverse community.
Qualifications include:
- High school diploma or GED
- Minimum of 3 years’ experience in a healthcare services role
- Possession of a valid driver’s license
