Community Health Engagement Coordinator Job at Family Health La Clinica
The Community Health Engagement Coordinator works with communities and local stakeholders (such as schools, healthcare providers, individuals, government officials, local agencies, and community action groups) to identify, develop, and implement strategies to successfully promote health to prevent injury or disease, build relationships, and help collect community data.
Collect and analyze data to identify community needs- Plan, implement, monitor, and evaluate programs designed to decrease health disparities, and improve the social determinants of health.
- Promote, maintain, and work to improve individual and community health by assisting individuals and communities in adopting healthy behaviors
- Enhance capacity, increase, and strengthen partnerships to reduce health disparities and improve health equity
- Deliver Community Health Outreach services as a member of Family Health La Clinica.
- Assist in managing grant funding for relevant projects and lead development of future funding initiatives.
- Support FHLC diversity, equity, and inclusion activities.
- Must adhere to FHLC policies and procedures.
- Regular and Reliable Attendance
- Other duties as assigned.
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. This position requires an individual, who has the ability to work independently, is able to work under pressure, is analytical and has excellent problem-solving skills. The ability to respect the confidentiality of patient information while performing job duties. Ability to establish and maintain effective working relationships with patients, employees and public. Experience with race, equity, and diversity, and health disparities projects is essential. Experience working with community coalitions and collaborations is required and exceptional facilitator skills are a plus.
High School diploma or equivalent required.- A degree in public health, nursing, community health education, community wellness, or closely related field with emphasis on health education preferred.
- At least three years relevant work experience, preferably in a community-based setting.
- Marketing, social media and web design skills preferred
- Significant travel throughout the region is required, although there are current limits to this due to the pandemic.
- Must be very self-directed and can independently organize his/her time to complete projects.
- Strong meeting facilitation and project management skills are needed.
- Grant writing and grants management skills are preferred.
- A good understanding of and commitment to improving the health of those who are underserved and living in rural communities.
- Valid Wisconsin Driver’s License required. Driving record must meet the requirements outlined in our company policy.
- CPR/BLS certification required within six months of hire.
