Responsibilities:
Under the supervision of Manager Community Outreach, the Community Health Educator is responsible for:
- · Identifying and partnering with providers and organizations to hold/participate in community events/health fairs/health education programs.
- · Identifying new community partners and maintains relationships with these partners in assigned service areas.
- · Speaking to members about plan specific programs to support their health and wellbeing.
- · Promoting member and public awareness of the importance and the availability of screenings and services in support of healthy lifestyles in efforts to improve HEDIS measures.
- · Identifying resources in the community for members to support the various needs of individuals on Medicaid.
- · Promoting proper use of the healthcare system to members.
- · Planning, developing, implementing and evaluating health education initiatives and programs throughout the assigned service area including signature programs such as Community baby Showers to help improve HEDIS, community gardens to reduce food insecurity and assisting at member screening events as required.
- · Coordinating and managing volunteer opportunities for employees of the plan.
- · Analyzing member event data to support impact of community based initiatives.
- · Coordinating and implementing face-to-face education and outreach sessions/workshops for members and potential members based on requests from community organizations.
- · Maintains and follows proper protocol for data collection and reporting.
- · Able to work a flexible schedule to include weekends, evenings and special events.
- · Travel throughout the assigned service area.
- · Informs members of the required actions to fulfill HEDIS, EPSDT, and Plan requirements and other referred outreach, through phone calls or home visits, and encourages them to comply.
- · Assists in addressing members’ barriers to care and to encourage/foster relationships with their medical/dental providers (medical/dental homes).
Travel is required in the following Pennsylvania Counties: Dauphin, Cumberland, Perry and Franklin. Candidates will be required to live within this geographic area of Pennsylvania.
Education/Experience:
- Bachelor’s Degree in Heath Education, Public Heath, Nursing or the combination of equivalent education/work experience.
- 1-3 years clinical experience, home health care.
- 1-3 years experience in health education, preferably in a public health environment that includes experience in dealing with disease specific/life style issues.
- 1-3 years Medicaid experience preferred.
- Medicaid managed care experience helpful.
- Valid driver’s license.
- Access to automobile w/current auto insurance required.
- Bilingual (Spanish Speaking) preferred
