Co-Responder Case Manager - CCBHC - Horizons - FT Job at Hutchinson Regional Healthcare System
Provides intensive, goal directed services to support individuals who have a mental health crisis and substance use problems result in the need for crisis stabilization as a part of the Crisis Response Team (CRT) Co-Responder Program with Hutchinson Police Department (HPD). The CRT Co-Responder Case Manager is paired with law enforcement officers in responding to calls involving behavioral health needs. The goal of this position is to aid with crisis resolution and de-escalation, referral and linkage to appropriate community services to avoid more restrictive levels of treatment and incarceration, and follow up to a crisis situation involving police post-crisis.

ESSENTIAL FUNCTIONS:
- Provides brief intervention case management utilizing identified and accepted methods to remediate symptoms of mental illness and/or improve emotional and functional well-being
- Provide crisis stabilization and de-escalation services to individuals experiencing a mental health crisis in the community in partnership with the CRT
- Conduct risk assessments as needed with the guidance of a Qualified Mental Health Professional to address safety concerns
- Participates in improving health outcomes and better utilize resources by diverting at-risk community members from future crisis, the criminal justice system and emergency rooms and towards behavioral health treatment
- Collaborates with community partners, caregivers, family members, physicians and other treatment providers regularly until the crisis episode is complete
- Support consumer achieve treatment goals, coordinate with collateral contacts, medication education, crisis management, prompt client to complete daily living activities, monitor client one to one for safety or as needed
- Contact service providers to request and arrange services; coordinate and monitor services rendered.
- Create and initiate plans to ensure clients attend mental health appointments
- Work towards successful reintegration of consumers back into their home, community, school and outpatient/community mental health services
- Schedule includes some night and weekend shifts as coordinated with the direct supervisor at HMHC and based on the needs recognized by Hutchinson Police Department’s data
- Improve community understanding of behavioral health needs and improved response through data collection and evaluation
- Complete all required documentation in a timely and accurate manner as defined by the agency.
- Other activities as assigned.
MINIMUM KNOWLEDGE AND SKILLS REQUIRED:

MINIMUM EDUCATION & EXPERIENCE REQUIRED:
- Bachelor’s degree or equivalent years of work plus experience in a related field.
PREFERRED EDUCATION AND EXPERIENCE:
- Bachelor’s degree in Social Work, Sociology, Psychology, or Special Education (with specialized training in human development and family dynamics).
- Three to five years’ professional experience in mental health services, crisis response, or a related field.
- Two years or more of experience working with educating and teaching clients about self-care, improving daily living activities.
LICENSURE OR REGISTRATION REQUIRED:
- Employee must complete CPST Case Management on-line training, KHS on-line training, pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens. Within 60 days of employment, the employee must have completed Adult Case Management training.
