ESSENTIAL JOB FUNCTIONS: *
- Understand the mission of Greater Portland Health (GPH) and its philosophy of integrated patient care and consistently comply with the policies and procedures as set forth in the Employee Handbook.
- Protect the rights of patients and families as defined in the patient’s rights policy and maintain patient confidentiality.
- Maintain and abide by ethical standards set forth by GPH, the National Association of Social Workers, and the Maine State Social Work Licensing Board.
- Evaluate patients in a timely and courteous manner.
- Appropriately document behavioral health care in electronic medical record.
- In partnership with the patient, clearly articulate a treatment plan in a timely manner and utilize appropriate resources when determining treatment plan.
- Participate in quality improvement programs that ensure patients living with HIV receive quality care in the office.
- Participate on quality improvement committees such as the weekly Ryan White Team meetings and the patient advisory council.
- Coordinate to ensure completion of daily huddles for administrative and clinical areas.
- When providing health and behavior assessment and counseling, develop a behavior change plan in partnership with the medical provider, healthcare team, and patient.
- Document in a complete and informative manner, timely psychosocial evaluations, assessments, and treatment progress notes using identified formats as directed.
- Follow through with commitments to patients and other providers in a timely and professional manner.
- Communicate plans and coordinate with other healthcare team members the scheduling of meetings with patients.
- Participate actively in social work team planning, implementation, and provide information as requested.
- Fulfill patient scheduling and productivity requirements.
- Maintain patient confidentiality at all times.
- Represent the organization in a positive and professional manner in the community.
- Give and receive feedback on effectiveness of clinical interventions through participation in daily healthcare team huddles, individual supervision and weekly team meetings.
- Contribute, educate, and assist other healthcare team members in understanding the patient and/or family problem from a behavioral health perspective.
- Design effective psycho-education curricula and lead effective psycho-education classes.
- Utilize time effectively and critically evaluation time utilization relevant to the effective operation of the social work services.
- Attend all mandatory safety related education programs, (fire, ergonomics, universal/standard precautions, etc.).
- Follow safety procedures to prevent injury to self and others.
- Interact harmoniously and effectively with others, focusing upon the attainment of organizational goals and objectives through a commitment to teamwork.
- Conform to acceptable attendance and punctuality standards.
- Complete other duties as assigned.
