POSITION DESCRIPTION
TITLE: Social Worker Professional II- Selkirk Neurology. Part-Time 8 hour per week. Required to be Licensed in state of Washington.
BASIC FUNCTIONS AND RESPONSIBILITIES
Social Work Professional serves as a member of the Selkirk Neurology, providing direct services to patients with Huntington's Disease and other genetic conditions, the patient's support systems, and providing consultation to Clinic medical staff on psychosocial issues, as well as providing direct services through the provision of psychosocial assessments, counseling, advocacy, education and referrals, and treatment planning. Experience in geriatrics, disability, or behavioral health preferred.
This Social Work Professional will demonstrate the knowledge and skills to provide care and/or services appropriate to the age of the patient in the service area.
CHARACTERISTIC DUTIES
This Social Worker will demonstrate competency in the provision of the following Social Work tasks within the identified service area, in concert with the competencies identified in the Core Competency Checklist.
1. Provide all needed social work tasks within the identified service area:
a) Review appointments on day of clinic to prepare for new patient assessments, or follow- up concerns of returning patients.
b) Consult with medical staff to screen patients for service needs, provide information regarding social work activity and to coordinate care as a part of an interdisciplinary team.
c) Provide in-person contact with patients and appropriate support in order to assess intervention needs and to provide services (i.e., counseling regarding adjustment to illness; patient and family education; assistance in patient/family decision-making; mental health and/or substance abuse treatment referrals; financial/disability advocacy; grief counseling).
d) Provide information and referral to patients in need of social service resources for financial and/or practical needs.
e) Provide case management to high-risk patients to ensure continuity of care, non- duplication of resources, and compliance with care.
f) Coordinate continued social work assistance for patients and families through the development of relationships with external social service programs.
2. Maintain communication with medical staff through documentation in medical chart in accordance with clinic standards. Serve as a Social Work liaison with the Huntington's Disease Society of America (HDSA).
As scheduling and priorities allow, assist with the development and facilitation of the Huntington's Disease Support Group if indicated. Assess for income, housing, legal, drug/alcohol, mental health, coping ability, support systems, insurance, advance care planning and spiritual concerns.
Provide crisis intervention when necessary, linking the patient to specialized resources. Consult with medical and nursing staff regarding social work activity, and to coordinate care as part of an interdisciplinary team.
3. Educational Role:
Educate about the social work role at Selkirk Neurology, and psychosocial needs of patient population. As a Social Worker in seminars, workshops, lectures as needed and when appropriate.
4. Departmental Duties and Responsibilities:
Participate as an active member of the Selkirk Neurology team, sharing responsibility for team facilitation via a model of rotation, and contributing to the successful functioning of the work team and department. Attend staff meetings, submit statistics on daily activity.
Maintain an environment that respects cultural, economic, ethnic, and lifestyle differences of patients, visitors, and staff.
Demonstrate accountability for own growth and competency. Participate in and seek out opportunities and activities which provide for continuing education, skill development, and self improvement.
Attend the annual Huntington Disease Association Meeting
Additional Job Functions:
- Assist in planning for and organizing quarterly Education Days
- We have established a Northwest HDSA & COE social work zoom group to meet every other month. Whoever fills the SW position there will be welcome & encouraged to join us!
- Collaborate with Washington HDSA Chapter MSW (Jane Zerabruk) on challenging cases & patients not yet connected to care.
- I have the option to lead regional or virtual support groups for patients, family, and/or caregivers. This will be an option for the new SW in your clinic, too.
- Complete and update comprehensive psychosocial assessments with patients whenever possible.
- Place referrals and manage resources.
- Assist with disability applications on local, state, and SSI/SSDI levels.
- Provide supportive counseling (brief intervention) for patients and caregivers. Assist patients and caregivers with short and long term planning.
- Provide education and assistance with Advanced Directives.
- Serve as a resource for clinic staff and providers around psychosocial barriers and social services.
- I participate in the Washington HDSA board meetings lx/month. These are not a mandatory but recommended

Pay: $28.00 - $33.00 per hour
