Summary:
Responsible for oversight of daily RPM operations to prioritize and distribute workload between team members.
This position supports the team in managing a subpopulation of patients with chronic conditions to optimize therapeutic regimens and provide individualized self-management support.
Responsibilities:
Essential Functions
- Monitors patient diabetes management plan and collaborates with primary care physician to optimize diabetes therapeutic regimen under collaborative practice agreement
- Utilizes new technology to assist with patient self-monitoring and care team remote glucose monitoring
- Assesses the healthcare educational psychosocial needs of the patient/family/caregiver and connects with appropriate resources
- Assists with care coordination for high-risk patients as needed and in collaboration with medical social worker
- Provides diabetes education services for individual and group education sessions depending on level of patient need or urgency
- Assists with care coordination needs for patients relating to diabetes care following transitions of care
- Participates in interdisciplinary care conferences with primary care providers and/or other care management teams to develop individualized plan of care
- Participates in ongoing program development and ensuring adherence to and promotion of ADA Standards of Medical Care for Diabetes
- Provides medication therapy management services to patients telephonically or in person as needed
- Develops professional working relationships with external health care providers and facilities
- Promotes effective collaboration with other Care Management teams and other health care providers
- Facilitates diabetes education session both during the day and in evenings
- Precepts University of Rhode Island PharmD candidates and PGY-1 pharmacy residents
- Complies with federal and local confidentiality laws including HIPAA ensuring patient privacy
- Adheres to Coastal Medical guidelines and policies for protecting patients� demographic clinical and financial information
- Performs other job-related duties as assigned

Education and Experience
- Current unrestricted pharmacist licensure in Rhode Island required
- Graduate of an ACPE accredited college of pharmacy with a BS or PharmD degree required
- Completion of PGY1 Pharmacy residency post-graduate training or equivalent clinical experience preferred
- Certification as Certified Diabetes Educator (CDE) or Certified Diabetes Outpatient Educator (CDOE)
- Knowledge of current diabetes technology (including insulin pumps and continuous glucose monitors) strongly preferred
- Excellent communication skills; both verbal and written
- Strong assessment critical thinking judgment creativity and decision-making skills
- Ability to work both on a team and independently
- Ability to use technology
- Ability to adapt to a fast-paced environment
- Ability to maintain quality safety and /or infection control standards
- Ability to perform standard clinical procedures according to established protocols
- Microsoft Office skills required
- Sit and stand for prolonged periods of time
- Work and move among all departments of Coastal Medical group
- Excellent hand/eye coordination
- Lift up to 15 pounds
- Manual dexterity to operate a personal computer or laptop as well as standard office machines and equipment as it pertains to one�s position
- State-wide travel and operation of motor vehicle may be required for offsite meetings and trainings
o Extensive clinical experience may be considered in lieu of formal training
o Or achievement of CDE/CDOE required within 1 year of hire date

o Must have a valid driver�s license and clear driving record
o Use of personal vehicle and proof of auto insurance is necessary
