Clinical Pharmacist Job at US Veterans Health Administration
Duties
The New York Harbor VA Healthcare system-Brooklyn Campus is currently recruiting for 1 Clinical Pharmacist (Staff Pharmacist). The incumbent is responsible for providing "expert" pharmacy oversight in the area of his/her responsibility. He/she supports all pharmaceutical care needs, direct or indirect, m his/her assigned area. Plans for and provides a wide variety of advanced pharmacy services relevant to the area assigned. Maintains the function of his/her assigned area to ensure compliance with OSHA, JCAHO and any other relevant regulatory body.
Some duties include but are not limited to:
Clinical
Reports adverse drug reactions and medication errors as they are identified. Participates in medication use evaluation. Makes rounds as appropriate, providing professional input and problem resolution as appropriate. Performs patient interviews and assessments as appropriate i n the area of assignment. Interfaces with providers on the uses of drugs in treatment regimens, patient safety, policy and cost. Serves as authoritative information source on drugs and their utilization in therapy. Maintains patient profile and recommends appropriate medication regimens in area of assignment. Documents all activities. Directs and/or participates in the preparation and distribution of pharmacy newsletters for patients, for physicians and for other health care professionals as needed.
Dispensing
Evaluates physician orders as to appropriateness based on patient medication profiles, inputs information , dispenses medication where appropriate according to policy and law. Maintains work load statistics on all aspects of dispensing process m area assigned as appropriate. Stores drugs and/or supplies in a professional and legal manner as appropriate.
Program Management
Accurately and completely prepares all reports, agendas, minutes of meetings, etc. required for the proper functioning of his/her area of responsibility, as directed by immediate supervisor and for Pharmacy Management in a timely manner. Evaluates program in area assigned, makes necessary recommendations for Pharmacy Management change and implements corrective action required to resolve identified problems. Develops and maintains an effective quality improvement program assigned area. Manages cost of drugs and supplies in area of assignment in an efficient, effective and accountable manner. Is responsible for all relevant aspects of medication usage in the assigned area.
Advanced Practice Functions
Perform discharge planning in a timely manner. Ensures that patients medications are made available timely and problems resolved. Manages all aspects of inpatient medication and related care in area of assignment. Prepares and coordinates all necessary forms and documents for clinical programs in the Admissions area. Prepares and coordinates an effective refill clinic using a multi-faceted approach for the New York Campus, ensuring full compliance with inspecting/regulatory bodies.
ADP Coordinator
Handles requests for complex data extraction in a timely, accurate manner. Prepares reports, displays data in an easy to use, easy to read format. Prepares annual ADP report, assesses staff education level and needs as well as equipment issues. Projects future equipment/education needs as part of this yearly plan.
Oncology
Serves as pharmacy hazardous spill consultant. Maintains safe, clean work environment. Properly stores and inventories chemotherapeutic agents.
Infectious Diseases
This pharmacist will work closely with the Infectious Diseases Team. The incumbent's responsibilities include but are not limited to:
a. reviewing patients' medications for appropriateness while monitoring efficacy and toxicity of medications and drug interactions
b. assessing patients' adherence to therapy plan and resolving conflicting issues with medications
c. providing patient-specific drug info, education and counseling
d. processing prescriptions according to "Scope of Practice"
e. adjusting medication dosage with respect to patient's condition when needed and according to "Scope of Practice
f. providing consults on medication alternatives, dosage adjustments, drug interactions and possible side effects common with prescribed medications
g. providing other Pharmacy service necessary for the proper function of the department.

Qualifications
Citizenship: US Citizen
Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). (This is required to supply if you have foreign education)
Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.
Physical Requirements: See VA Directive and Handbook 5019.
English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d)
Creditable Experience
Knowledge of Professional Pharmacy Practices. To be creditable, the experience must have demonstrated the use of knowledge, skills, and abilities associated with professional pharmacy practice. Professional practice means paid/non-paid employment as a professional or unlicensed graduate pharmacist as defined by the appropriate licensing board.
Residency and Fellowship Training. Residency and fellowship training programs in a specialized area of clinical pharmacy practice may be substituted for creditable experience on a year-for-year basis. The pharmacy residency program must be accredited by the American Society of Health-System Pharmacists (ASHP). A fellowship program that is not accredited by the American College of Clinical Pharmacy (ACCP) will need to have comparable standards for experience to be creditable (Professional Standards Board refers to the Deputy Chief Consultant for Professional Practice for the determination).
Quality of Experience. Qualifying experience must be at a level comparable to pharmacy experience at the next lower level. Experience as a Graduate Pharmacist is creditable provided the candidate was used as a professional pharmacist (under supervision) and subsequently passed the appropriate licensure examination.
Part-time Experience. Part-time experience as a professional pharmacist is credited according to its relationship to the full-time workweek. For example, a pharmacist employed 20 hours a week, or on a 1/2-time basis, would receive 1 full-time workweek of credit for each 2 weeks of service.
Grade Determinations.
In addition to the basic requirements for employment above, the following criteria must be met when determining the grade of candidates.
GS-12 Clinical Pharmacist (Full Performance Level)
Experience or Education. Candidates must meet one of the following:
1. 1 year of experience equivalent to the next lower grade level, or
2. Completion of an ACPE-accredited Pharm.D. program.
Assignment. A pharmacist in this assignment handles routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations. These include, but are not limited to: reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; providing oversight of technical staff in all aspects of medication distribution.
Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs):
1. Knowledge of professional pharmacy practice.
2. Ability to communicate orally and in writing to both patients and health care staff.
3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
