A registered and licensed Rhode Island Pharmacist, this person will work with physicians, nurse practitioners and nurse care managers, and other members of the clinical staff as part of a primary care medical care team focusing on quality care, patient safety and reduced costs. The Clinical Pharmacist provides patients and clinicians with therapeutic assessments, medical recommendations, and guidance regarding common chronic diseases.

Duties & Responsibilities:

• Embedded within the care teams at multiple clinical sites
• Provides clinical pharmacy consultation as part of interdisciplinary care plans for complex patients
• Assists in the selection of the most cost effective, evidence-based regimens for patients with common and complex conditions; Assists with monitoring patients on certain medications with appropriate lab testing and dosage adjustments
• Serves as expert medication resource for providers and support staff at all PCHC sites
• Monitors patient response to treatment, drug/drug and drug/disease interactions. Assesses medications prescribed at dosages higher than approved by the FDA, as well as any medications prescribed “off-label” by the medical team

Qualifications:

• Experience working as a pharmacist in a clinical setting
• CPR required
• Must be proficient in working with electronic health records
• Ability to effectively communicate, and interact with the patient population of PCHC. Responsible for maintaining patient and family confidentiality in accordance with HIPPA.
• Expert understanding of FDA, and other Federal, state and local regulations related to pharmacy
• Ability develop and maintain accurate and streamlined processes and procedures
• Previous exposure to formulary management preferred
• Bi-lingual preferred

Education:

Graduate of an accredited School of Pharmacy, and current licensure in the State of Rhode Island as a Pharmacist.
