Premera offers an HMO option for its members, and an important part of that experience and care is providing clinical pharmacy support. The Clinical Pharmacist contributes to the development and delivery of clinical programs that ensure clinically appropriate and cost effective medication use through direct outreach to members and academic detailing to providers. The clinical pharmacist further supports drug utilization management strategies and programs for Pharmacy, Medical Management Services, and HEDIS Clinical Quality measures.
What you’ll do:
- Provide direct outreach & clinical support for members & providers related to optimizing medication management for HMO members
- Utilize prescription and medical claim data to generate clinical recommendations for medication management and retrospective Drug Utilization Review (DUR)
- Provide clinical recommendations pertaining to, but not limited to, gaps in care, high risk medications, compliance and adherence, drug interactions, therapeutic substitution, and generic substitution
- Support new member onboarding process, partnering with case managers and customer service agents to identify and respond to member clinical inquiries and needs.
- Assist case managers in identifying and eliminating barriers to care.
- Provide telephonic consultation and, as necessary, visits to provider clinics and member homes.
- Help design drug therapy components of disease management algorithms.
- Recommend appropriate interventions to meet accreditation requirements (NCQA, HEDIS, etc.).
- Utilize health plan’s formulary information to guide appropriate medication recommendations
- Active participation in various clinical departmental meetings and forums to improve clinical programs, enhance processes, and share clinical information
- Continuously look for ways to streamline and improve workflow
- Assist with user acceptance testing for new clinical programs and system enhancements.
- Assist Pharmacy leadership in analyzing and researching pharmacy cost trends and clinical program opportunities within the HMO
- Support and serve as a preceptor for pharmacy students and/or residents, introducing them to drug utilization management and care management activities.
- Perform other duties as assigned.
What you’ll bring:
- Pharmacist Degree (Required)
- Active US Pharmacist license in good standing (Required)
- Clinical pharmacy training, clinical residency training from an accredited Pharmacy program (Required)
- Two (2) years’ experience as a pharmacist (Required)
- An advanced degree (e.g. PharmD, MBA, MHA, MPH or equivalent experience)
- Experience in ambulatory care delivery
- Experience in providing telehealth services directly to patients
- Completion of PGY-2 Ambulatory care residency program
- Experience in managed care setting for one year or more

The pay for this role will vary based on a range of factors including, but not limited to, a candidate’s geographic location, market conditions, and specific skills and experience.
National Salary Range:
$100,100.00 - $170,200.00
National Plus Salary Range:
$113,100.00 - $192,300.00
- National Plus salary range is used in higher cost of labor markets including Western Washington and Alaska.
