Essential Functions:
- The primary responsibility of the Clinical Liaison (LIAISON) is to facilitate a seamless transition for patients discharging from a facility setting to the care of an LHC Group LTAC for post-acute care needs. The LIAISON will respond to referrals and will complete a full pre-admission screening of the patient. In addition, the LIAISON will continually communicate between healthcare providers during all phases of transition. The LIAISON role is aligned with company market development initiatives. This clinical liaison understands their role in growth, serving more patients and delivering exceptional care. Using discretion and independent judgment, the LIAISON identifies specific referral target audiences and develops referral-building strategies in collaboration with the Administrator. The LIAISON will interact with all departments and services as needed to complete the referral/evaluation process and address barriers to admission by working effectively with referral sources and the Admissions Team.
Formal Education: Associate Degree or certification equivalent
Experience: 1 year
Experience Requirements
- One (1) year acute care hospital
License Requirements
- Must have independent means of transportation, possess a valid driver's license and the ability to
- Automobile LIABILITY insurance coverage must be
- Current RN, LPN/LVN or RT license in Louisiana
Skill Requirements
- Must have computer skills with knowledge of spreadsheet and word processor
- Clerical and business machine skills and knowledge of business
- Good organizational and personal communication
Skill Desired
- Understanding of LTAC Industry
