Job Title: Clinical Case Manager /LPHA
Pay Rate: $30-35
Job Summary: Provides mental health services for clients within the Good Samaritan Shelter system of care. Provides services as an LPHA for clients participating in substance use treatment programs.
Responsibilities:
· Conducts biopsychosocial mental health assessments to determine treatment needs of clients.
· Determines the client’s eligibility for Mental Health treatment.
· Provides rehabilitative and psychotherapeutic counseling; individual, couples, group and family therapy/rehabilitation as determined based on treatment plan created in consultation with the client and work to empower them to secure living, vocational, recreational, and interpersonal goals.
· Provides evaluation and assessments of individuals with substance use disorders to complete medical necessity forms for treatment programs.
· Develops mental health and/or substance use treatment and service plans that include the client and family members.
· Documents clinical activity presenting diagnostic and interview findings, clinical evaluations, and recommendations.
· Cooperates and works with other agencies, including Santa Barbara County Department of Behavioral Wellness, Child Welfare services, Public Health and probation with proper releases, to secure good outcomes.
· Educates clients and their families regarding their diagnosis, treatment and recovery process.
· Provides Case Management to assist clients to obtain stable housing, physical health care, benefits and entitlements, and to learn and develop living skills.
· Other duties as assigned.

Education and Experience:
· Registration with the California Board of Behavioral Sciences as an LCSW or LMFT or Registration with the California Board of Behavioral Sciences as a MFT Associate or Associate Social Worker and:
· Possession of a master’s degree in social work or marriage and family therapy from an accredited university; or,
· Possession of a master’s degree in marriage, family and child counseling, marital and family therapy, counseling psychology, clinical psychology, counseling with an emphasis in marriage, family and child counseling, or social work with an emphasis on clinical social work or other equivalent master’s degree from a college or university approved by the California Department of Consumer Affairs, Board of Behavioral Science Examiners.
· Experience working with individuals and families with mental health issues, substance use addiction, homelessness and/or legal issues;
· Must be proficient in Microsoft Word and Excel. Knowledge of HMIS, ShareCare and Clinician’s Gateway is a plus
· Have reliable transportation and:
· A valid driver’s license
· The ability to qualify for Good Samaritan Shelter insurance coverage
Successfully complete the following as a condition of hire:

Work Environment
The employee may be in contact with individuals and families in crisis who may be ill, using alcohol and drugs, and who may not be attentive to basic personal hygiene, health and safety practices. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and lack of personal hygiene. The employee must be ready to respond quickly and effectively to many types of situations, including crisis situations and potentially hostile situations. The noise level in the work environment is usually moderate in an office setting. Sometimes work may become stressful when working under pressure.
