Clinical Account Manager - Palliative (Remote) Job at CareCentrix
Overview:
As the Clinical Account Manager - Palliative Care you will be responsible for the clinical support and monitoring of the Palliative Illness Management (PIM) population health programs in their designated region. This position will monitor, plan, coordinate and manage daily clinical operations to facilitate the achievement of program goals and key performance metrics. Oversight includes accountability to build and maintain relationships with network provider leadership, program managers, and other stakeholders.
Responsibilities:
- Works closely with the SVP, Account and Clinical Management, Regional Director of Clinical Services and the business development team to achieve the key performance metrics of participating network providers in the designated market
- Develops and maintains strong trusting relationships with network provider leadership and program managers
- Provide program orientation training, ongoing clinical coaching and support of the network provider program manager which may include consulting on staffing, member engagement and population management strategies.
- Daily, weekly and monthly monitoring of quality and clinical metrics and benchmarks of each assigned program as outlined by Turn-Key’s quality and performance improvement program.
- Lead clinical discussions in weekly case management meetings with a focus on assessing workflow and clinical pathways, identifying problems and opportunities for improvement
- Provide clinical support at monthly network provider meetings in reviewing program clinical metrics, program goals, benchmarks and key performance indicators
- Identifies opportunities for process improvement and staff education to produce optimal outcomes and communicates any concerns or challenges to their manager.
- Work in concert with network provider to develop program improvement plans
- Conduct and support training and on-boarding of new network provider program managers
- Coordinate and support ongoing clinical training provided for new PIM team members
- Perform supportive field visits with PIM team in coordination with network provider program manager
- Bachelor''s degree in nursing. RN license required.
- 5 years experience in hospice, palliative care or related community-based healthcare services
- Experience in hospice, palliative care or other related community-based complex population health management programs
- Solid understanding of the post-acute care space including palliative care organizations, hospices, health plans, hospitals, home health organizations, nursing homes, and assisted living
- Advanced Microsoft skills with the ability to develop presentations and documents for internal and external audiences
