Children's Case Manager Job at Suncoast Center Inc
Job Duties:
1. Meets with the client to accurately and rapidly assess and document the clients symptoms, diagnosis, and interventions, and assures that all program criteria are met for opening the case.
2. Completes all documentation to open the client into Targeted Case Management.
3. Completes an accurate and up to date assessment, functional assessment and service plan within 30 days of the clients case being opened, and every six months thereafter.
4. Assists clients and their families in developing realistic, effective and measurable care plans. Establishes prioritized behaviorally descriptive, realistic desired changes with clients, and develops a realistic plan for their accomplishment with appropriate time frames.
5. Maintains a Continuity of Care in accordance with Florida Statutes and Florida Administrative Codes, 65E-15, Case Management Regional Operating Policy and Florida Medicaid Manual.
6. Acts as an Advocate for the client and their family at all relevant staff meetings, court hearings, school meetings etc. or arranges for a substitute.
7. Links the client and family with necessary resources to assist with the clients needs and monitors the effectiveness of these interventions.
8. Uses appropriate crisis intervention skills and anticipates and prevents crisis where possible.
9. Provides services in a way that utilizes the principles of recovery, and treats every client as a partner in a process to better their lives.
10. Remains current in the general knowledge of psychotropic medication, side effects, and DSM-V diagnosis, and recognizes when medical services are appropriate.
11. Complies with Medicaid standard for required face to face contact for children receiving targeted case management services.
12. Meets productivity standard of 70%.
13. Utilizes support funds effectively to meet the needs of the clients.
14. Completes all progress notes and other documentation (i.e. time sheets, outcome measures, CFARS/FARS, etc.).

Minimum Qualifications
Education: A bachelor's degree from an accredited college or university in any field.
Experience: At least twelve months of experience providing related case management services to children and families. Must be able to assist families struggling with multiple stressors. Understanding of the special problems and treatment of consumers with mental illness and child welfare issues is essential. Understanding of community resources and social support development is required.
Additional Requirements:
1) Completion of AHCA approved targeted case management training within 3
months of hire.
2) Must be certified through the Florida Certification Board (FCB) as a Behavioral Health Case Manager within 6 months of hire.
3) Must be able to complete a level II background screening and pre-employment drug test in compliance with Florida Law.
4) Must be at least 21 year of age.
5) Must be able to provide official transcripts
Working Conditions: Both in-office and in the community. Schedule may be irregular if emergencies arise.
Computer literate
Must drive personal vehicle and carry agency required minimum auto insurance
