Job Highlights:

Demonstrates knowledge in providing grief and loss support. Coordinates the delivery of spiritual services with the hospice interdisciplinary team. Maintains compliance with policies, procedures, State and Federal regulations, and accreditation standards.

Qualifications and Skills:

  • Assess the patient for spiritual needs and implements interventions based on patient goals.
  • Completes a comprehensive assessment and plan of care within 5 days of admission.
  • Participates in interdisciplinary team meeting to coordinate and revise the plan of care.
  • Facilitates visits by local clergy, pastoral, or other local faith leaders as requested by patients.
  • Educates the patient and family on hospice services and the role of spiritual support.
  • Communicates effectively with the members of interdisciplinary team.
  • May conduct religious services of prayer and worship at the request of patients and families.
  • May participate in providing bereavement services.
  • Documents at point of care; documentation is accurate and reflects patient decline.
  • May provide spiritual support to hospice employees and other community members when applicable.
  • Completes education, participates in education in-services, continuing education, seminars, and /or self-study
  • Maintains compliance with policies, procedures, State and Federal regulations, and accreditation standards.
