The certified aide is a trained para-professional, who is under the specific supervision of appropriate professional staff. The aide acts in strict accordance with the written Plan of Care to provide supportive services to clients. The supportive services include personal care, homemaking, and meal preparation services. The services assist a client and their family in the achievement of physical and emotional comfort.
JOB DUTIES/KNOWLEDGE: (50%) 1. Consistently provides personal care services, such as reading and recording temperature, pulse and respiration. Also assists the patient with a bed bath, sponge, tub or shower bath. The aide may shampoo the patient's hair in the sink, tub, or shower. In addition the aide provides nail and skin care, oral hygiene, and assistance with toileting and elimination. The aide assures maintenance of a clean, safe and healthy environment that is conducive to maintenance of health and safety of the client.2. Applies safety principles and proper body mechanics to performance of specific techniques of personal and supportive care such as ambulating of client, transferring clients, assisting with normal range of motion and positioning, and doing household chores.3. Performs supportive services, such as homemaking, and other environmental services, which facilitate receiving sanitary and safe care at home.4. Prepares and provides adequate nutrition and fluid intake, including providing medically prescribed diets under supervision5. Observes reports and documents changes in client status or expectations of service promptly to Ultimate One Home Health Care supervisor or professional.6. Understands basic elements of body functioning and reports changes in client body functions as indicated.7. Teaches client and cues client to activities that will increase/sustain client independence and that the aide is qualified to teach.8. May perform specific duties as delineated under a medically supervised plan of care.9. May performs escort service for errands and transporting client to appointments, etc. for private pay and ADvantage clients10. Recognizes emergency situations and implements appropriate emergency procedures when indicated.11. Utilizes good infection control procedures in relation to performance of duties. Follows agency procedures in all instances.12. Uses equipment and supplies responsibly. Takes care to prevent spill and breakage incidents in the home.13. Documents care provided accurately and submits visit records and mileage logs as required.14. Respects rights of client including the client's right to privacy and respects the client's property; adheres to Ultimate One Home Health Care confidentiality policy and maintains professional objectivity.15. Attends inservice according to Ultimate One Home Health Care policy. Meets agency requirements annually.

Qualifications:1. Six months to one year prior experience as a nurses' aide in a hospital, nursing home, and/or home health environment, or successful completion of nurse aide course that meets state and federal requirements.2. CNA and/or Home Health Aide certification current with the State Nurse Aide Registry.3. Able to take oral and written instruction, and to observe, records and makes reports.4. Has a phone or pager and transportation.5. Able to work cooperatively and/or independently6. Good communication skills.7. Knowledge and abilities: a) Must be able to demonstrate the knowledge and skills necessary to provide care to and communicate with a primarily geriatric population. b) Must be able to demonstrate appropriate to job duties, knowledge of the principles of growth and development over the life span. c) Must be able to assess data reflective of a client's status and report the appropriate information relative to the client's age-specific needs.
DEGREE OF TRAVEL:
Home visits daily. Periodic office meetings. Must have reliable transportation and agency required liability insurance.
DEGREE OF DISRUPTION TO ROUTINE, OVERTIME:Must be able to adapt to client status and needs as directed. Schedule changes daily, due to staffing, client condition and needs, new clients, etc.SAFETY HAZARDS IN JOB:Possible infections from clients, auto accidents, unsafe neighborhoods.PHYSICAL DEMANDS:Must be able to sit, stand, walk, stoop, carry loads of 25 or less pounds, bend. Normal auditory and visual acuity. Must be able to communicate verbally and in writing.ENVIRONMENTAL EXPOSURE:Possible exposure to infections, hazardous wastes, toxic chemicals, extreme weather conditions
