The Targeted Case Manager’s purpose is to identify the medical, social, educational and other needs of the eligible family, identify the services necessary to meet those needs, and facilitate and coordinate access to those services. The Targeted Case Manager is responsible for the overall management of assigned caseloads of children/families served by KidsPeace New England.

Schedule: Part Time (< 30 hours/week), Primarily M-F with additional hours & weekends as needed
