Responsibilities:

� Provide case management rehabilitation crisis interventions and outreach services in home office and community settings to adults or children with mental illness

� Collaborate with psychiatric & allied mental health teams in assessing diagnosis intervention & treatment planning

� Conduct substance abuse treatment with clients

� Provide clinical interventions & linking Clients with community resources such accessing medical & dental care affordable housing financial benefits developing support networks and applying ADL skills in needed real-life environments

� Conduct comprehensive clinical assessment & develop interventions for substance abuse sexual abuse symptom management and family support & education

� Coordinate emergency psychiatric services within a timely manner including documenting interventions alternatives considered and response of Client/Client�s Family and supporting agencies

� Participate in treatment & discharge planning

Requirements:

� Bachelor�s degree in the Human Services field such as Social Work Psychology and Rehabilitation Services preferred

� Experience working with adults with substance abuse sexual abuse or involvement with the criminal justice system preferred

� Valid driver�s license and insured vehicle

Shift & Schedule: M-F 8am-4:30pm (Full-Time 40 hours/week)
