Job Summary:
The Residential Habilitation Professional (QIDP) manages a caseload of residential participants with developmental disabilities. Works with the participant’s team to develop a unique plan of service and works with Renewed Horizons Direct Support Professionals to ensure that plan is implemented fully throughout the individual’s plan year, making updates and changes as needed.
Minimum Qualifications:
- Education and Experience. To be qualified as a Residential Habilitation professional, a person must:
- Have a Bachelor’s Degree in Psychology or a human service-related field.
- Have at least (1) year of experience professionally supervised with the population served; and experience writing and implementing behavior and skill training program plans; or
- Provide documentation the employee received such training from an experienced Residential Habilitation professional; and demonstrate the ability to write and implement behavior and skill training program plans
- Criminal History and Background Check. A Residential Habilitation professional must have satisfactorily completed a criminal history check in accordance with IDAPA 16.05.06, “Criminal History and Background Checks.”
- First Aid and CPR Certification. A Residential Habilitation professional must be certified in First Aid and Cardio-Pulmonary Resuscitation (CPR) appropriate for the age of participant they serve prior to providing direct service to participants and maintain current certification thereafter.
- Must be able to perform the responsibilities of a Residential Habilitation Professional. (See below)
- Direct Service Qualifications. The Residential Habilitation professional at times will be expected to provide direct support services to participants and must meet the qualifications of direct support professional.
Job Duties/Responsibilities:
- Case Management. Responsible for integrating, coordinating and monitoring each participant’s Individual Support Plan (ISP), including but not limited to, complete age-appropriate functional assessments, develop participant program plans according to the current authorized plan of service, monitoring participant progress and documenting changes as necessary to ensure revisions are made for progress, regression, or inability to maintain independence.
- Supervise Habilitation Services. Review of direct services performed by direct service employees to ensure that employees are implementing the programs as written and demonstrate the necessary skills to correctly provide the services.
- Quality Assurance. Monitor participant’s monthly documentation (Daily Service Logs, Program Plans, Medication Assistance Record, etc.) looking for direct service employees’ errors and getting corrections to ensure documentation is accurate and complete.
- Skill Training. Provide all skill training to agency direct service employees necessary to fulfill each participant’s plan of service. (For new and current employees when there are changes to the participant’s plan of service)
- On-Call. Participate in an on-call rotation to assist the Shift Supervisors and direct service employees with after hour emergencies.
- A Part of Bigger Teams. Works alongside with other agency professionals, as well as with their individual’s person-centered planning teams, to support our residential participants.
