Job Description – Case Manager

Case Manager will have a caseload of around 5 families. Case Manager will ensure safety and stabilization of families assigned to them. The position is 35- 40 hours per week, full-time, with evenings and weekends required as needed. The overarching goal of Case Manager is to avoid out-of-home placement of youth on their caseload.

Duties – Case Manager

· Case Manager will make home visits to family on caseload and assess safety

· Case Manager will provide on-call services to family

· Case Manager will conduct crisis intervention with family as needed

· Case Manager will assist family in creating a plan to improve family functioning

· Case Manager will provide parenting skills counseling, stress management, anger management, and communication skills development as needed

· Case Manager will connect the family with community resources

Requirements – Case Manager

Bachelors Degree in related field with valid driver’s license and own vehicle

AND

At-least 2 years of experience working with children and/or adults with behavioral health/mental health disorders

Job Type: Full-time

Pay: $16.00 - $21.00 per hour

Schedule:

• 8 hour shift
• Weekend availability
