If so, we’d love to connect with you! We are Mental Health Cooperative (MHC), and we are looking for a Care Manager - Spanish Speaking to join our team! You will provide continuous treatment team care management services to consumers with severe and persistent mental illness.

To join our team, you’ll need:

• Ability to multi-task consumers’ needs to ensure individualized care plan goals are met
• To be an active listener who can identify obstacles, and conceive solutions for the development of individualized care management plans and ability to speak Spanish fluently
• Compassionate response to consumers’ needs while maintaining healthy boundaries
• Collaborative spirit working with team members to meet consumers’ needs
• Ability to transport consumers to appointments in a safe and reliable manner
• Bachelor’s Degree in a Behavior Health Field

We’re especially looking for someone with social work experience within a mental health clinic, school, agency, or hospital environment who is:

• Dependable to both consumers and team members
• Flexible to changing priorities and needs, especially during crisis
