Responsibilities:

This position offers a challenging and exciting opportunity to be part of an innovative statewide program designed to integrate children’s physical, behavioral, and social health in creative and collaborative ways. It will afford you the opportunity to enhance your written communication and time management skills, connect you to a wide array of community providers in the medical, psychiatric, and educational fields, and allow you to make a tangible positive impact on one of our community’s most vulnerable populations.

The Care Manager will:
  • Carry a caseload of children with varying levels of physical, behavioral, and/or social health challenges.
  • Provide comprehensive assessment and reassessment of youth as needed inclusive of medical, behavioral, rehabilitation, social service and long term care needs.
  • Develop and regularly amend crisis planning and person-centered plans of care with the youth/family, incorporating feedback from the youth’s interdisciplinary care team.
  • Facilitate communication between care team members.
  • Foster collaboration with family and care team to ensure that services are delivered and treatment recommendations are understood and followed.
  • Connect youth/family with referrals to community resources to support the youth’s health and wellbeing.
  • Meet standards and deadlines for paperwork as established by Agency, Health Homes, and Department of Health.
Qualifications:
Education and/or Experience Required at Entry: A Bachelor’s of Science or Arts degree or NYS licensure and current registration as a Registered Nurse with two years relevant experience; or a Master’s Degree with one year relevant experience required. Demonstrated ability to work with people of diverse backgrounds required. Degrees in Health or Human Services field preferred; experience working with an electronic health record system preferred. Experience working with children and families preferred. Thorough working knowledge of local and regional public and private resources needed.
