The Care Management team at Salem Health thrives on collaboration; seeks individuals who share a desire to be life learners, and those not afraid to help guide patients through difficult life challenges. It will take being flexible enough to work independently problem solving the needs of one patient, while alternatively participating in a multi-disciplinary team of skilled professionals for another. The Care Management Associate is a professional member of an interdisciplinary team working with assigned patients, completing tasks that assist in utilization management, patient care and transitions. You will provide general patient/family/community education regarding community resources and health care services as assigned.

Background/Expertise:

• Bachelor Degree required.
  
• One (1) year medical setting using similar skills and performing similar activities preferred.
