Broadstep, Inc. is hiring a CCS Care Coordinator to provide assistance and leadership to our programs and staff in Milwaukee, WI.
Brief Description:
CCS is an exciting program offering lots of growth and expansion! Care Coordinators will be a part of this growing system of services within Milwaukee County. In addition to essential duties, Care Coordinators will have the opportunity to identify needed but lacking services in the community and assist in their development as desired. The care coordinator in a consistent figure in the lives of the consumers they serve. The Care Coordinator is involved in the progress of the client from the initial assessment to the discharge from the program. The Care Coordinator meets regularly with the consumer and the service providers and natural support people who work closely with them. The Care Coordinator receives regular consultation from a mental health and/or substance abuse professional.
Duties:
- Establish eligibility using a Mental Health/AODA Functional Screen
- Work with the consumer to identify and build a recovery team of professional and natural supports
- Facilitate recovery meetings with the team to identify and prioritize goals, with particular focus on the clients voice
- Assist consumers in identifying services from the CCS service array appropriate for addressing current barriers to recovery
- Broker services as needed with providers in the CCS network
- Follow-up with service providers to ensure quality care and goal progression
- Complete necessary authorizations and paperwork
- Appropriately document all encounters
Qualifications: Bachelor’s preferred, Valid Driver's License
Required Experience: 1 year experience working with mental health preferred
