Addressing Cancer Together (ACT) expands access to life-saving breast cancer screening, diagnosis, and treatment. Through coordinated collaboration of healthcare providers, funders, nonprofits, volunteers, and patients with lived experience, ACT is saving lives and reducing the financial burden of cancer.
Who we want:
The Cancer Care Coordinator is an essential part of the United Way for Greater Austin team implementing Addressing Cancer Together (ACT). In this role, you will ensure that eligible individuals seeking breast cancer preventive screenings, diagnostics, and treatment are able to access the services they need. You will serve as a resource and advocate for ACT patients (individuals with no health insurance) diagnosed with breast cancer in Williamson, Hays, and Bastrop counties. You will process referrals, case-manage, and support ACT patients using a human-centered practice lens to help these individuals navigate a complex health system in Central Texas.
The Care Coordinator is results-oriented and maintains high moral and ethical standards. You must be self-motivated, meet deadlines, work independently, and make sound decisions. The ability to work well with diverse populations is a must. You will ensure effective, timely, and quality communication reflecting care and concern for each individual with whom you speak while maintaining patient confidentiality and quality care.
What you’ll do:
- Receive and process patient referrals and confirm eligibility.
- Provide case management, including navigation, coordination of clinical care with patients, healthcare providers, and other partners
- Identify and refer patients to other health and social services as needed to support the well-being of those we serve
- Support patients to access patient assistance programs as well as enroll in health insurance.
- Support administrative functions of the program, including billing, data collection, and reporting
- Track and monitor patient progress in a database system
- Serve as an advocate for patients navigating health services, insurance, and billing departments
- Receive and follow up on calls, emails, and messages from patients, healthcare providers, or insurance providers
- Comply with organizational guidelines and health care laws and regulations
- Other duties as assigned by your supervisor

What you’ll bring:
- Bachelor's Degree in a Social Science field, with 1-year experience or a minimum of 2 years experience with care management in a healthcare environment
- Ability to read, speak, write, and communicate effectively in both English and Spanish
- Clear communication skills (verbal and written) with the ability to communicate with patients and professionals at all levels
- Flexibility with an ability to adapt and adjust to change
- Ability to practice case management with professionalism and confidentiality, adhering to HIPAA standards as required
- Demonstrated willingness and ability to ensure patient and organizational goals are the primary priority
- Ability to track and monitor patient progress in a database system
- An ability to prioritize work to meet daily and monthly program goals
- Ability to work from home, including reliable access to the internet
- Ability to travel with your own transportation as needed during work hours

This position is located in Austin, Texas, and reports directly to the Vice President for Addressing Cancer Together.
