Scope of Work:
FAR GROUP is currently requesting high-quality qualified laboratories to perform biological screening and DNA analysis on evidence for certain forensic cases being investigated by the
Salt Lake City Police Department. Services will be requested on an as-needed basis.

Responsabilities:
SLCPD cases shall be available for review at any time. The responsibilities and tasks required of the
Candidate in providing such services include, but shall not be limited to, the following.
Evidence Handling.
Candidate shall provide Chain of Custody (COC) forms, and establish sample acceptance procedures that are mutually acceptable to the Candidate and the City. City may, from time to time communicate specific instructions and requests to the Candidate concerning the performance of the work, which is the subject hereof. The management of the work, including the exclusive right to control or direct the manner or means by which the work is performed, remains with the Candidate.
- Candidate shall accept evidence samples by appointment during normal business hours of
Monday – Friday from 8 AM to 5 PM, excluding holidays. All evidence samples shall be delivered to Candidate by SLCPD personnel.
- Candidate shall provide on-site forensic casework consultations, allowing investigators to meet with the forensic scientists in person to review the evidence for best evidence submission.
- Case items shall be individually and securely packaged under seal with the items for each case grouped together.
- An item-by-item submission form/manifest shall be included with each delivery of case items from the SLCPD. Candidate shall compare the submission form/manifest with items received and inspect all evidence seals within one business day of receiving the samples. Candidate laboratory shall report any manifest inconsistencies or problems with package or seal integrity immediately upon receipt of a shipment/or delivered samples from the SLCPD.
- An item-by-item chain of custody shall be maintained by Candidate laboratory starting with the unique identifier on the container received from the SLCPD. A chain of custody shall be maintained on all sub-samples and DNA extracts starting at the time that they are generated by Candidate laboratory. All items, sub-samples and extracts shall be uniquely identified and traceable to the SLCPD case number and item number.
- All items of evidence and sub-samples shall be securely stored under seal when not in use.
DNA extracts shall be securely stored when not in use.
- After analysis, all case items, sub-samples and DNA extracts shall be returned to the SLCPD.
All items shall be securely and individually packaged under seal with the items for each case grouped together. The Candidate laboratory will notify the SLCPD that cases/samples are
ready for pick-up.
- Candidate shall maintain a complete electronic and/or paper chain of custody for all samples.
- At no time will Candidate consume more than 50% of any item or sub-sample for DNA extraction unless written permission is first obtained from SLCPD explicitly stating so.
- Cases with the oldest date of receipt by Candidate shall be analyzed first. Upon request by SLCPD, Candidate shall test a case out of receipt order.
- The samples shall be identified throughout the testing process with a case and item numbers as provided by SLCPD. Candidate may utilize its own barcode so long as the barcode is associated with only one sample.
- Candidate shall provide, in writing, any problem and associated corrective action regarding samples.
Screening for Biological Evidence
- Candidate shall screen evidentiary items as requested by the SLCPD in the documentation accompanying each case. Screening can stop once the biological substance of interest is identified on a probative item unless otherwise noted in the case documentation.
- All items shall be assessed visually and notes should be kept that can include but are not limited to the following: 1) general appearance of the item, 2) location and appearance of all probative stains, 3) descriptions of defects or unusual observations.
- T. esting of all samples will be using current techniques and standards.
- All items or stains testing positive for the biological substance of interest that are retained for DNA analysis should be removed from the bulk item, uniquely identified and packaged separately as a sub-sample traceable to the bulk item and case number.
DNA Testing
- . Tests shall include STR,Y-STR and Investigative LEAD analysis. *In addition, newer testing technology methods included are: a) Forensic investigation Genetics Genealogy. b) Next Generation Sequencing. c) Rootless Hair Genetic Full Genome Sequencing.
- . The amount of human DNA in all evidentiary DNA extracts shall be evaluated prior to amplification.
- . Data and documentation of control and ladder results for all samples within a case shall be provided to the SLCPD at the very least, electronically.
- Documented procedures for contamination and sample mix-up control should be followed including the comparison of all profiles with those of Candidate laboratory staff and any identified contamination profiles. Instances of contamination or sample mix-ups and the associated troubleshooting and/or corrective actions shall be documented in the case file of any effected samples.
- Stutter peak assignments shall be as determined by the vender laboratory’s validation studies. Major and minor profile designations can be made in mixed profiles as determined by Candidate laboratory validations.
- Any ambiguous peaks that are above the RFU threshold and fall within the allele-calling region shall be evaluated by re-injection and if still present, evaluated by re-amplification.
- All micro-variants shall be re-injected or verified by additional confirmative results and tri-alleles shall be confirmed by re-amplification and reported.
- Spikes that are in more than one color or outside of the allele calling range may be interpreted from one injection. If a spike is present in only one color and within the allele calling range, the sample should be re-injected to demonstrate it is not reproducible.
- A random match probability shall be calculated as prescribed by the National Research
Council 1996 “The Evaluation of Forensic DNA Evidence” using a validated and relevant database on single-source profile matches with probative reference standards and on major profiles of interest in a mixture that are consistent with probative reference standards.
- A combined probability of inclusion shall be calculated as prescribed by the National
Research Council 1996 “The Evaluation of Forensic DNA Evidence” using a validated and relevant database on mixtures with no clear major and minor components or on mixtures where the profile of interest is a minor component.
- Reasonable efforts shall be made to obtain results, improve marginal data, troubleshoot unexpected DNA results, evaluate and confirm micro variants, tri-alleles, spikes and ambiguous peaks. Efforts can include, but are not limited to re-injection, re-amplification, re-quantification and re-extraction (consuming no more than 50% of the sample without expressed written approval from the SLCPD). These activities will be done at no extra cost to the SLCPD. All troubleshooting and reanalysis methods shall be validated by the
Candidate lab. Documentation for all troubleshooting, instances of contamination, sample switches, corrective actions and re-analyses associated with a sample shall be contained, at a minimum within the respective electronic case file.
- Upon request by SLCPD, Candidate shall cooperate fully with the Utah
State Crime Laboratory in the sharing of test procedures, test results, and other information of common interest related to a specific case. Candidate shall use
test kits that are compatible with Utah State Crime Laboratory standards for analysis and
CODIS upload.
- Data Reporting.
- Candidate shall provide a written report detailing the biological screening and DNA testing performed and the results of that testing which will include an interpretive statement for any DNA comparisons with known standards supported by statistical calculations. Each case shall include an associated data package and all data files that contains the following:
Hard copy of the written report and/or electronic copy, if available.
Hard copies and/or electronic copy, if available, of any and all of the following that pertain to the given case:
- Pictures
- Worksheets
- Notes
- Quantitation data and results
- Genescan® and GenoTyper® files
GeneMapper ID® currently not accepted but may be at a future date during
the term of the agreement.
- DNA profile results in table form along with any editing notes or analyst over- ride documentation
- Any re-analysis, trouble-shooting and/or corrective action documentation
- Clear indication of what data sets and controls were used for the final allele calls and if any composite profiles are reported.
- Candidate shall provide completed case files to the SLCPD within 60 days from receipt of evidence with rush service available upon request.
- Candidate shall provide formal pre-trial DNA case consultation and trial expert witness testimony services.
Confidentiality. Case information and test results are confidential criminal justice information.
Access to case information and evidence, including sub-samples and DNA extracts, must be controlled by vendor’s laboratory and limited to those personnel identified to the SLCPD as working . Vendor’s laboratory cannot provide information concerning cases to anyone other than specified representatives of the SLCPD. Any outside inquiries related to the processing of samples shall be immediately reported to the SLCPD designee.
