Job Overview:
The Bilingual Community Health Worker (CHW) is a full-time position with the Training & Capacity Building (TCAP) department and reports to the Program Manager. The TCAP department serves as a community-clinical integration hub for delivering, scaling and sustaining evidence-based chronic disease self-management and prevention programs for at risk, disparate populations such as African American and Latino/Hispanic populations, and those living with disabilities. Ultimately, TCAP’s work aims to simplify and increase access to evidence-based self-management programs and to encourage healthy behaviors for community members to prevent illness as well as identify resources to alleviate social determinants of health.
The CHW serves as a vital link between Health Promotion Council (HPC) programs, the community served, and the health and social systems of care. This bilingual, Spanish-speaking, staff member is a frontline public health worker who has a close understanding of the communities they serve. This trusted relationship enables them to serve as a link between physical and behavioral health and social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.
The Bilingual CHW works to ensure that the program practices:
- Respect for patient and family values, preferences, and expressed needs;
- Listens to the patient and family voice and use this input for planning and promoting success;
- Actively engage patients and families to collaborate in goal setting, decision making, health-related behaviors and self-management, and
- Assess and communicate in the preferred language, at an appropriate literacy level, and in a culturally appropriate manner.
The CHW’s responsibility will be to support, promote and recruit for chronic disease and prevention programs by engaging people in their communities by tabling and presenting at local health fairs; faith-based events; canvassing in neighborhoods; and supporting with company Town Hall Meetings and other virtual or telephonic events. The CHW will also be responsible for facilitating chronic disease and prevention program workshops for community members, both in-person and virtually. To facilitate workshops, in accordance with Self-Management Resource Center’s (SMRC) fidelity requirements, the CHW will need to be trained as a lay leader, and possibly Master Trainer, through SMRC.
The CHW role is contingent upon funding. This position may evolve to support HPC’s broader community and home-based initiatives, as funding and points of intersection across HPC are identified.
Responsibilities:
- Plan, develop, implement, coordinate and track promotion and outreach to the community and partners such as tabling at health fairs and events, and canvasing in the neighborhood.
- Conduct education and training sessions within the communities served with an emphasis on increasing disease self-management and prevention behaviors.
- Develop and maintain current knowledge on delivering evidence-based programs and how to engage communities through culturally relevant practices, as assigned.
- Assist in the development and dissemination of promotional materials and tools, including presentations, to increase awareness of evidence-based self-management and prevention programs.
- Schedules, prepares for, and co-facilitates, if qualified as a Master Trainer, lay leader trainings to train new lay leaders in the Self-Management Resource Center (SMRC) programs delivered by HPC, completing all required documentation for data collection and SMRC licensing.
- Conducts fidelity assessments on new lay leader facilitation for SMRC programs and completes reporting.
- Identify target audiences and conduct outreach and education to community members promoting vaccine confidence as well as evidenced based programs.
- Routinely identify resources, webinars, and new literature to assist the TCAP team and HPC staff in staying current with chronic disease related news and evidenced based programs. Circulate the relevant resources to appropriate individuals.
- Facilitate referrals and appointments for services, including health care, social, housing, or other services, based on the clients’ needs.
- Monitor HPC’s referral hub email, toll-free phone number, and web form for new referrals from healthcare providers or self-referred community members.
- Conduct phone-based follow-up calls, as needed, with clients to provide personalized education.
- Help community members understand their health condition(s) and develop strategies to improve their health and well-being.
- Log and document all contacts with members. Support Project Manager’s monthly, quarterly, and annual reporting of process measures to project funders by ensuring tracked data is up-to-date, accurate, and complete.
- Maintain confidentiality of client information and data.
- Attend weekly supervision calls or meetings with Program Manager.
- Participate in teleconference and in-person meetings with project team and project funders as appropriate.
- Assist in the preparation of grant requests and funding continuation.
- Learn and utilize HPC’s server for storing and retrieving files.
- Obtain HIPAA training within 3 months of employment.
- Other duties as assigned.
Skills:
- Fluent in specified language and possessing a profound understanding of specified culture.
- Strong oral and written communication skills in English and Spanish, including public speaking and presentations.
- Excellent understanding of the health system, community-based organizations, and partners in designated service area(s).
- Experience successfully managing competing timelines.
- Ability to establish priorities, and work both independently and in a team environment to meet objectives with minimal supervision.
- Excellent problem solving, conflict resolution and time management skills.
- Excellent interpersonal skills and ability to effectively interface with partner organizations and community residents.
- Availability during daytime and some evening and weekend hours as needed
- Travel throughout Philadelphia County is required (and surrounding counties, as needed); has valid and current driver’s license and automobile insurance
- Detail-oriented, with excellent organizational skills.
- Proficient in Microsoft office suite.

Experience and Qualifications:
Required:
- Fluent in Spanish, with strong oral and written communication skills.
- Five years of experience in public health or social service-related work.
- At least three years experience working with diverse populations, low-income individuals, individuals from different religious backgrounds.
- At least three years experience conducting community health education programs.
- CHW certification or obtained within first year of employment
Preferred
- Certified Self-Management Resource Center Lay Leader, and possibly Master Trainer, in Chronic Disease (CDSMP), Chronic Pain (CPSMP), Diabetes (DSMP) Self-Management Programs (English and Spanish versions), preferred or willingness to become certified as a lay leader within 4 months of hiring.
- At least three year experience delivering CDMSP, CPSMP, DSMP (English and Spanish).
- National Diabetes Prevention Program Lifestyle Coach certification, preferred.
- Trained in Motivational Interviewing within 5 months of employment
Education Requirement:
- High School Diploma
- Preferred, bachelor’s degree in public health, social work, or related field; or equal relevant work experience.
