Description
We are currently recruiting Behavioral Health Consultants to join our team! As part of patient-centered health at CCP, this position provides integrated behavioral health services to patients currently receiving primary care services within the patient-centered medical home model. The behavioral health consultant (BHC) will in an integrated team-based model of care, providing behavioral health consultation to patients in order to improve psychosocial functioning and health. This position is Monday - Friday.
Providing assessment, consultation, and brief intervention for psychological/psychiatric, behavioral, medical problems and/or disorders and behavioral intervention in chronic disease management.
In collaboration with behavioral health program management, the BHC will also provide orientation, consultation, and ongoing training to pertinent primary care staff as needed in order to strengthen the relationship between primary care and behavioral health services and ensure a team-based model of health care delivery.

Annual salary range: $64,637.87 - $96,956.80
CCP salary ranges are designed to be competitive with room for professional and financial growth. Individual compensation is based on several factors unique to each candidate, such as work experience, qualifications, and skills. Some roles may also be eligible for overtime pay.

Requirements
A minimum of a master’s degree in mental health counseling or social work leading to independent licensure as a mental health provider (LMHC/ LCSW) from an accredited institution. Current New York State licensure or license eligible.
Ability to communicate clearly and effectively with multiple members of the patient care team.
Ability to work within an electronic medical record, good documentation and written skills.
Desire for on-going learning, training, and professional development a plus as we place value on helping you grow as a clinician.
Desire to work with an interdisciplinary team and to be a leader in healthcare reform. Prior experience in integrated care is a plus!
Previous experience working with adult and children/adolescent within ethnically and culturally diverse families.
