We have one full-time opening for an Administrative Assistant (Referral and Linkage Navigator) :

Job Title:
Referral and Linkage Navigator

Home Office:
Vienna or Cairo IL – can be hybrid – at least 2 days in office per week is required

Area of Focus:
Behavioral Health

Program(s):
Behavioral Health - Outpatient Mental Health Programming helps individuals build life skills and well-being through behavioral health counseling, psychiatric care, and other care coordination and support services.

Immediate Supervisor:
Office Administrator or Behavioral Health Assistant Director – depending on if applicant will provide direct services

Hours of Work:
40 hours/week

Minimum Qualifications:
High School Diploma or equivalent, Associate’s Degree in Human Services Field, Bachelor’s Degree in Human Services Field is preferred (Bachelor’s level applicant job duties may differ based on experience and credentials)

Skills & Knowledge:
Knowledge of local health and human services resources; Excellent written and oral communication skills; Excellent computer skills; Ability to navigate enhanced phone system and electronic health record (EHR); Facilitates communication across agency staff and applicable programs to ensure continuity of care and timely follow up for individuals accessing behavioral health services; Case management skills, Ability to interpret, adhere, and implement standards associated with program rules, regulations, and contract deliverables; ability to work with and provide high quality services to individuals from all walks of life.

Special Requirements:
Must complete required registry clearances and criminal fingerprint background checks, maintain and provide proof of a valid driver's license, maintain and provide proof of personal vehicle liability insurance, must be age 21 or older, Maintain First Aid & CPR certification, must be skilled in the delivery of care coordination services, travel to all agency sites when appropriate.

Job duties include, but are not limited to:
  • Participates as a member of the management team to enhance and integrate total agency function; attends management meetings and other agency committees or special projects as assigned; displays initiative by seeking out responsibility.
  • Bachelor’s level applicant is preferred to keep a small caseload in SUPR program and perform DUI/Risk Education
  • Assists as needed with setting up transitions in care and appropriate linkages for those entering or exiting higher level of care.
  • Screens referrals for programmatic appropriateness.
  • Maintains centralized scheduling for clinical outpatient services
  • Manages no-show management for clinical outpatient services.
  • Manages waitlist(s) for clinical outpatient services.
  • Help manage and coordinate psychiatric internal referrals including communication with Behavioral Staff, clients, and Psychiatric Team.
  • Send out letters/texts/e-mails for appointment reminders.
  • Assist with obtaining signed documents for proper care of clients.
  • Completes various administrative forms to assure timely and accurate data collection in accordance with agency policies and procedures.
