POSITION: ACT Team Leader -Licensed Clinical Social Worker (LCSW) / Associate Clinical Social Worker (ASW)
SHIFT: Varied days/hours (*rotating on-call hours)
LOCATION: CCBHC at South Bay Family Recovery Center – Gardena, CA
* this is a field-based position
Under supervision of the CCBHC Project Director, the ACT Team Lead-LCSW/ASW is the clinical supervisor of the team, providing a combination of clinical, case management, leadership, supervision, program evaluation and CQI services to establish, support, continuously improve and maintain a high-performing, high-fidelity Assertive Community Treatment team serving adults with serious and persistent mental illness and co-occurring substance use disorders in support of each person’s individual recovery journey.
ASW must have a minimum of 2250 total supervised hours
KEY RESPONSIBILITIES:
· Participates in daily/weekly ACT team treatment planning meetings within the CCBHC.
· Attend all ACT team meetings in order to facilitate an integrated, multidisciplinary team approach to service.
o Daily Team Meeting – to organize work for each day and to review recent contact with each client.
o Treatment Planning Meeting – weekly or bi-weekly to develop and review treatment plans/decisions.
o Clinical Supervisor Meeting – 1 to 2 times monthly for training, education and professional development of ACT team staff (the ACT Team Leader will occasionally lead this meeting).
· Functions as a lead clinician providing structured face-to-face therapeutic interventions to consumers with severe and persistent mental illness to provide support and guidance in all areas of functional domains: adaptive, communication, personal care, domestic, psychosocial, problem solving, etc. in preventing, overcoming, or managing the recipient's level of functioning and enhancing his/her ability to remain in the community.
· Consults with staff physicians, nurses, and other para-medical personnel regarding medical management of patients
· Works closely with CCBHC project director on overall quality improvement of ACT services.
· Provide rotating On-Call/After-hours crisis response services.
· Participates in medical and services utilization audits, peer review and quality
assurance, and CQI activities as requested.
· Other duties as assigned.
QUALIFICATIONS:
· A graduate from a university with nationally recognized accreditation with a Master’s degree in a related human service field
· Must possess and maintain an appropriate license to practice as a Licensed Clinical Social Worker (LCSW) in accordance to the California BBS
OR
· Must possess and maintain an appropriate license to practice as a registered Associate Clinical Social Worker (ASW) in accordance to the California BBS
** ASW must have a minimum of 2250 total supervised hours
· Must have a strong commitment to the rights and ability of each person with a severe and persistent mental illness to live in normal community residence; work in market jobs; and have access to helpful, adequate, competent, and continuous supports and services.
· Skills and competence to establish supportive, trusting relationships with persons experiencing severe mental illnesses and co-occurring substance use disorder. Respect for individual rights and personal preferences in treatment are essential.
· Recovering individuals must have a minimum two years clean, sober and abstinent to be considered for employment.
