Diabetes Self-Management Education and Support (DSMES) Manager
The Fund for Public Health in New York City, (FPHNYC) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers. To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives implemented by DOHMH to advance community health throughout the city. It facilitates partnerships, often new and unconventional, between government and the private sector to develop, test, and launch new initiatives. These collaborations speed the execution of demonstration projects, effect expansion of successful pilot programs, and support rapid implementation to meet the public health needs of individuals, families, and communities across New York City.

PROGRAM OVERVIEW
The Bureau of Equitable Health Systems (BEHS) is a bureau in the Division for Center for Health Equity and Community Wellness at the NYC Department of Health and Mental Hygiene located at 42-09 28th Street, Long Island City, NY. The Healthcare System Innovation and Support Team (HSIS) within BEHS operates NYC REACH, helping New York City providers to implement new health care delivery models and quality initiatives, participate in payment reform programs, and connect to community and health resources to support their patients. POSITION OVERVIEW
BEHS and HSIS are seeking a DSMES Manager to support strategies to increase the availability and utilization of diabetes self-management programs for New Yorkers with diabetes. This position would work closely with the Agency’s DSMES program, accredited by the Association of Diabetes Care and Education Specialists (ADCES). These strategies include but are not limited to:
  • DSMES Program Accreditation
  • Oversee and expand DOHMH’s ADCES-accredited DSMES program and patient-facing services
  • Ensure the program continues to meet ADCES standards and maintains its accredited status
  • DSMES Delivery
  • Deliver in-person and virtual DSMES workshops utilizing BEHS’s developed curriculum
  • Coordinate with Agency and community partners to schedule and recruit for DSMES workshops
  • Develop and implement quality improvement initiatives based on best practices and lessons learned
  • Oversee consultants assisting with DSMES delivery
  • DSMES Stakeholder Engagement
  • Assist in coordination of DSMES stakeholder education and networking events to promote sharing of best practices and discussion of barriers to increase DSMES efficacy and sustainability in New York City. DSMES stakeholders can include service providers and organizations or individuals interested in delivering or supporting DSMES.
  • Contribute to event development, promotion, and facilitation, including program scheduling and agenda planning, coordination with guest speakers, and documentation of program outcomes.
  • DSMES Umbrella Accreditation
  • The DSMES Umbrella Accreditation Program allows aspiring diabetes educators in high need areas to receive DSMES accreditation, with fewer barriers and additional support for sustainability, as a part of BEHS’s own DSMES program.
  • Support team in continued development and implementation of the Umbrella Accreditation program to increase the number of participants and improve the services provided to enhance program efficacy.
QUALIFICATIONS
  • Bachelor's Degree in nutrition, public health, or related field
  • Current registered dietitian (RD) credentials in New York State
  • Certified Diabetes Care and Education Specialist (CDCES) or Board Certified - Advanced Diabetes Management (BC-ADM) credential, a plus
  • 3+ years experience in delivering diabetes self-management education and medical nutrition therapy in outpatient settings, particularly in group sessions
  • Experience in delivering DSMES across diverse patient populations
  • Familiar with ACDES-accreditation and/or ADA-recognition for DSMES programs; Experience in designing or managing a DSMES program, a plus
  • Flexible, outgoing, and able to work independently and productively without direct daily supervision
  • Excellent communication and organizational skills
  • Knowledge of and proficiency in Salesforce and Microsoft Office, including Excel, PowerPoint, and Word
  • Bilingual in English and Spanish, preferred
  • Must be willing to travel within the 5 boroughs of NYC
ADDITIONAL INFORMATION
This role will be based at DOHMH located in Long Island City, Queens, though travel to sites across the city may be needed. There is the potential of shifting to a hybrid work schedule. There is a potential for this position to transition to DOHMH, for which eligibility requirements include NYC residency.

SALARY AND BENEFITS
FPHNYC offers a comprehensive benefits package. The salary range for this position is $75,000-$85,000, commensurate with experience.
